Pellucid marginal degeneration looks a little different than keratoconus. But a good elevation map is an even better place to start. When looking at a keratoconic eye topographically, an axial map that shows a central invagination, superior flattening and inferior steepening is a good starting point. Keratoconic eyes usually have a Fleischers ring, a Munsons sign or Vogts striae, as well as central corneal scarring. Never forget: Keratoconus is a non-inflammatory thinning disorder that is paracentrally located. Here are three characteristics to look at that will help you distinguish the two: Pellucid marginal degeneration and keratoconus look very similar, especially topographically, so they are easy to confuse. Differences greater than 1 are another indication of possible keratoconus. This criterion takes into consideration that the superior/inferior index should be different between the two eyes. This is where the Rabinowitz/McDonald criterion becomes useful. Remember, you also must note the difference in curvature between the right and left eyes. So, you get a high superior/inferior index that artificially indicates keratoconus, even though the patient has contact lens-induced corneal warpage. However, high-riding rigid contact lenses tend to flatten the superior cornea and steepen the inferior cornea. Many O.D.s diagnose keratoconus based on steep K readings and a difference in the superior and inferior quadrants of more than 1.50D-2.00D. Specifically, it indicates that keratoconic eyes have K readings greater than 47.00D, and inferior/superior index values between 1.4 and 1.9. The Rabinowitz criterion is another measure of corneal change associated with keratoconus. This comes in handy, especially when treating a patient with keratoconus. The greater the toricity, the higher the number. Normal values for this index range from 41.25 to 47.25. This is not a true K reading per se because its a value that is averaged over the toric apex, not necessarily over the corneal apex. A number less than 1 indicates a good relationship between the two values, which means you can, indeed, use mathematical formulas to approximate the shape of the cornea. A reading greater than 1 indicates a poor relationship between these two shapes, or greater degrees of corneal irregularity. Basically, it is the best fitting relationship between a toric ellipse and the actual corneal shape. This statistical index lets you know if you can match a mathematical model of what the cornea should look like to what the cornea actually looks like. Some topography units create both elevation and axial maps as well as statistical indices to let you know whats wrong and why.Ĭorneal irregularity measure. Keratoconic eyes demonstrate high prolate shape factors, and corneal warpage topographies typically have low shape factors that are either oblate or just barely prolate. In such cases, the center of the cornea is flatter than the periphery. If the cornea is oblate, the topographer will produce a negative shape factor value, which means the normal corneal symmetry has been reversed. A positive value indicates the normal prolate-shaped cornea, which means the cornea is steeper centrally and flattens toward the periphery. If eccentricity and shape factor decrease, youll see the opposite effect.įurther, the positive and negative values let you know whether you are dealing with a prolate or an oblate shape. As eccentricity and shape factor increase, you get a greater rate of corneal flattening, decreased sagittal depth and a less spherical surface. Anything in between is considered a normal range of corneal eccentricity. Anything below 0.15 is a low shape factor. Anything above 0.5 is a high shape factor. Shape factor is similar to corneal eccentricity. This is one of the most useful indices to help you differentiate between contact lens-induced corneal warpage and a case of true keratoconus. You can identify these patients fairly easily using these statistical indices. Armed with the ability to definitively distinguish between contact lens-induced warpage, keratoconus and pellucid marginal degeneration, youll be able to better and more quickly fit irregular corneas.ĭo not mistake contact lens-induced warpage for keratoconus patients with the former are simply a sub-population of normal individuals who have a greater degree of corneal irregularity than most. It is only January, yet 2023 has opened with 3 tremendous albums – ‘Chaos and Colour’ from Uriah Heep, Iggy Pops ‘Every Loser’ and King Krakens debut ‘MCLXXX’.Įach one is a potential contender for album of the year, yet each is musically diverse from the other. So don’t delay, get on and order or buy your copy now, although leave one for me! Staggeringly, on top of all that, the entire album could be played in the live environment, theres no filler anywhere to be heard! It is an excellent album that rewards repeating playing, ideally at a decent volume! The band capture that pure enjoyment of making music perfectly. The album is full of familiar sounding songs, yet they capture a sense of urgency and freshness that still keeps the band relevant for a potential new audience. Having been around for over 50 years, it is unsurprising that Uriah Heep are able to produce a new album, but perhaps what will be exciting for existing fans is that they are able to to produce dramatic new material like this. There’s a great guitar solo and you can visualise the band throughly enjoying themselves as they hammered this one out in the studio. Keyboards pump along and the song – like the entire album – is full of optimism. ‘Closer to Your Dreams’ is the final track on the album, and its a full on rocker in the mold of ‘Easy Living’. In places the song really motors along (but elsewhere drops away to almost silence) and quite honestly, yes, it will be a fantastic live set piece to highlight the individual members of the band. ‘Freedom to Be Free’ is however, over 8 minutes long, and fills that time with great vocals and fantastic instrumental breaks from the band. It is just under 4 minutes long and so perhaps live we will hear a longer version or maybe I am just getting greedy! Synth and guitar breaks intertwine and the song almost ends too quickly. ‘Fly Like An Eagle’ gives us guitar, synths, drums and bass driving a mystic tale of meditation powered by Bernie Shaws vocal. An album highlight, this is another classic Uriah Heep piece, really kicking into gear at about the 4 minute mark when heads should shake and fists punch the air before dropping back again to round out the song. ‘You’ll Never Be Alone’ clocks in at just under 8 minutes, the song ebbs and flows whilst it tells its story. This is one of those majestic Uriah Heep songs and guess what – I bet it sounds fantastic live! ‘Golden Light’ picks up the pace again with those keyboards from Phil Lanzon pumping out the riffs alongside Mick Boxes guitar. I have little to add to that except considering it is over 7 minutes long, it holds the attention in the most delightful way. ‘One Nation, One Sun’ is described as ‘ a journey of soaring balladic contemplation’. Sometimes less is more, and this song is fits that perfectly. Mick Box gives us a great guitar riff as ‘Hurricane’ opens up. Whilst I am repeating myself this should also be fantastic live and its certainly one of my favourites on the album but I should warn you now, this is an album chock full of great music! Drumming from Russell Gilbrook and bass from from Dave Rimmer underpin the song. ‘Age Of Changes’ kicks off with vocal harmonies and canters along at a fair pace that would put many other bands to shame. This is classic Heep and if your stereo is not turned up by now then you know what to do – turn it up to 11! Mick Box delivers a great guitar solo and this song should be fantastic live. ‘Hail the Sunrise’ has plenty of keyboards from Phil Lanzon swirling throughout a song about stone circles and the solstice. ‘Silver Sunlight’ keeps the pace going, Bernie Shaws vocals are tremendous (as they are throughout the album) and the whole song screams ‘second single’! The challenge to other bands is still there, but this a band sounding triumphant and there is no point in resisting, just let the sound wash over you. Expect the airwaves to be lit up as this blasts from your radio or listening device and shame on any radio stations that don’t play it! A catchy tune and lyric fully justifies it’s use as the debut single. This does not sound like a band going through the motions, but more like a band throwing down a challenge to all comers. ‘Save Me Tonight’ kicks off the album and is a suitably full on opening number. git, indicating that Git is tracking the files in the directory. I've been doing multiple calibrations on the two cheaper ones, to try to find a combination where all three match visually, but none of them have a lot of banding. One is high quality, the others are cheaper. I have use three monitors on my desktop machine. So you might decide to relax the target values in order to reduce the banding. If the target you chose (white point, luminance, gamma, etc.) required a large change in the LUTs, you might see unacceptable posterization and banding, even if the color measures close.
